What It Actually Means
Cuate is Mexico’s warm word for a buddy or close friend — and one of the few slang words in this dictionary with an Aztec passport. It comes from Nahuatl cóatl, meaning “twin” (and also “snake” — Quetzalcóatl is the famous relative), and it still means “twin” in Mexico today: hermanos cuates are fraternal twins. From “twin” to “someone as close as a twin” to “buddy” is a short, rather beautiful walk.
The Warmth Scale
Mexican Spanish grades friendship finely: an amigo is a friend, a cuate is a buddy you actually hang out with, a carnal (literally “of the same flesh”) is brother-level. Cuate sits in the comfortable middle — warmer than amigo, less intense than carnal, and safe in almost any casual context. Buen cuate as a description (“good people”) is one of the nicer compliments a Mexican can pay someone who isn’t even present.

Common Pitfall
Almost none — cuate is clean, warm, and hard to misuse, making it ideal early-deployment Mexican slang (unlike, say, everything involving pedo). The only footnote is geography: it’s understood across Central America but reads as instantly Mexican; Spain says colega, Argentina says amigo/chabón. If a Mexican calls you cuate, take it as the small badge of acceptance it is. If they upgrade you to carnal, you’re in for life.
